A 12-week support group for people learning to live with the emotional and everyday impact of chronic health conditions.

Program Description

Living with a chronic condition can change more than your health. It may affect your plans, energy, relationships, responsibilities, and the way you see your body. Even when other people cannot see what you are carrying, the emotional weight can still be very real.


The Chronic Conditions Support Group offers a guided space to talk honestly about those changes with people who understand. Over 12 weeks, members will explore emotions and coping, grief and body trust, self-compassion, personal values, communication, relationships, responsibilities, and ways to move forward.


The therapist will use education, reflection, and practical exercises, with a primary focus on Acceptance and Commitment Therapy techniques. The goal is not to pretend the condition is easy or to force positivity. It is to help you make room for difficult feelings, reconnect with what matters to you, and feel more confident navigating life with greater flexibility and support.

Who It Is For

This group may be a good fit if you are adjusting to a new chronic health diagnosis or still struggling with the ways a long-term condition has changed your daily life.


Chronic conditions may include autoimmune diseases, gastrointestinal conditions such as Crohn’s disease, and other ongoing health concerns that cannot simply be cured and may affect your energy, relationships, work, routines, or sense of self.


You do not have to have everything figured out before joining. This group is for people who want support understanding the emotional impact of chronic illness and building more confidence as they navigate life with their condition.


Each participant will complete a short screening process so we can make sure the group is appropriate for your needs.

Eryn Dorsey

Eryn Dorsey brings experience in hospice, grief support, and group counseling. Her work with grief is shaped by years of supporting individuals and families through some of life's hardest transitions. In this group, Eryn's role is to guide the conversation, support the group process, and help create a respectful space where grief can be spoken without shame.

FAQs

Do I need to be newly diagnosed to join?
No. The group may help people who recently received a diagnosis as well as those who have been living with a chronic condition and are still adjusting to its impact.


What conditions are included?
The group is designed for people living with ongoing health conditions that are not curative and can make daily life more difficult. This may include autoimmune diseases, gastrointestinal conditions such as Crohn’s disease, and other chronic conditions. The screening process will help determine whether the group is a good fit for your needs.


What will we work on in the group?
Topics include understanding the emotional impact of chronic illness, coping with grief and changes in your body, responding to difficult thoughts with more self-compassion, reconnecting with your values, communicating your needs, and managing relationships and responsibilities.


What is Acceptance and Commitment Therapy?
Acceptance and Commitment Therapy, often called ACT, offers practical ways to respond to difficult thoughts and feelings while continuing to make choices connected to what matters most to you.


Why is there a screening process?
The screening process helps protect the group experience and makes sure participants receive the right level of support.
